Yes, definitely. Some people may not want to give their phone number, but just get around that by making it an optional field.

On number 4 – I've been thinking about adding a request for a phone number on the contact form for my website. Seems like not having the phone number of an interested prospect can be a detriment If they want to schedule a phone appointment.
